As a patient at the Ottawa Fertility Centre you have the responsibility to :

• Be sure that you understand your proposed treatment, its success rate and the risks involved with your fertility treatment
• Follow instructions given to you by the staff of the Ottawa Fertility Centre
• Report any ill effects, unusual symptoms or events
• Give medication injections as instructed and dispose of needles and syringes in a safe manner
• Ask questions of staff if any instructions are not clear, or if you are uncertain of what you are asked to do (do not guess)

As a patient at the Ottawa Fertility Centre you have the right to :

• Be informed of the cause(s) of your infertility and available treatment choices
• Choose which (if any) treatment you will attempt, as long as it fits within the standard of care and the policies of practice at the Ottawa Fertility Centre
• Determine how you will use your eggs, sperm and embryos
• Change or withdraw your consent to treatment at any time
• Have questions answered within the limits of scientific or medical knowledge
• Be treated with dignity, fairness and respect
